This level includes weekly linen laundry service, assistance with bathing, showering, dressing, and grooming up to twice a week, and diabetic management oversight. It also provides monthly vital sign checks, weight monitoring, self-medication assessments, semiannual wellness assessments, and coordination of physician visits and lab work 1-2 times per month. Oversight of third-party provider services is also included.
Offers two loads of personal laundry, assistance with bathing or showering up to three times weekly, and dressing and grooming twice daily. Residents receive daily reminders for mobility, redirection, and to attend meals and activities. Specialized diets are self-managed, while medication management includes up to two passes per day with diabetic oversight.
This level also includes wellness services that provide vital sign checks and weigh-ins up to twice a month, semiannual wellness assessments, monthly self-medication assessments, and coordination of physician visits and lab work. Residents undergoing active chemotherapy or radiation therapy may qualify for this or a higher level of care, depending on treatment frequency.
Includes personal laundry up to four loads per week, assistance with bathing or showering up to four times weekly, and dressing and grooming assistance up to three times daily. Residents receive reminders and redirection up to three times daily, with personal escorts to meals and activities for both walking and wheelchair use. Mobility assistance is provided up to three times daily, along with a simple meal setup for specialized diets. Continence care is available up to three times daily, and medication management includes up to four passes per day. Diabetic management includes insulin administration, and finger sticks up to twice daily.
This level also includes wellness services, providing vital sign checks and weigh-ins up to three times per month, along with semiannual wellness assessments, monthly self-medication assessments, and coordination of physician visits and lab work. Short-term monitoring is available post-procedures such as dialysis, chemotherapy, or radiation. Routine wellness checks are provided once in the morning and once in the evening upon request.
Provides personal laundry of more than four loads per week, bathing or showering five or more times weekly, and dressing and grooming assistance more than three times daily. Residents receive reminders, redirection, and mobility assistance more than three times daily, with personal escorts to meals and activities. Diet and mealtime assistance includes monitoring and help with eating, while continence care requires staff attention more than three times daily.
Medication management includes more than four passes per day, and diabetic management covers insulin administration and finger sticks more than twice daily. This level also includes wellness services, providing vital sign checks and weigh-ins more than three times per month, along with semiannual wellness assessments, coordination of physician visits, and lab work. Increased wellness checks are available, with routine check-ins more than once in the morning and evening or as needed for added safety.
